Pregnancy Care Planner

Content Supplied by NHS Choices

The more you know about your pregnancy and your options, the more you are likely to feel in control. The information given here is based on The Pregnancy Book, which your midwife should give you at your first appointment.

Maternity Care

There is usually no medical need to see your GP during your pregnancy.

If, however, you have any questions not answered here or any concerns, please do book in.

Once you have confirmed you are pregnant 6 weeks pregnant (this is calculated using the first day of your last period as day 1) you will need to complete the on-line self referral form which can be found by visiting the Poole Hospital Web page https://www.poole.nhs.uk under maternity services. The form will ask you questions about yourself, your family and your medical background to ensure you receive  the most appropriate care. The midwives will then arrange your booking appointment prior to your 12 week Ultrasound scan. Any problems with this then contact reception.
