New Patient Registration
If you live within our practice area you are welcome to register with us.
You will need to complete the New Patient Registration Forms as well as a GMS1 form which can be given to you by Reception.
If you have any medication on repeat, it would be a good idea to provide us with a list of the medication you require.
We normally ask new patients to allow us 48 hours to get them onto our system however we do understand that in some cases this is not possible and an urgent registration may be required.
When handing in your registration forms, we will ask for a form of photo ID and an ID that shows your current address. This could be any of the following:
- Driving Licence
- Utility Bill
- Bank Statement
If you feel like you are unable to provide any form of ID please let Reception know when handing in your registration forms.
